Use MyChart to access your medical records

MyChart is a secure service for MultiCare patients that makes accessing your medical information simpler and more convenient than ever. Whether you’re at home, at work or on the go, you can get test results, prescription details, appointment information, after-visit instructions summary notes and more. Log in or sign up to view your medical information on MyChart.

If you have an active role in managing someone else’s health care, whether it’s your child (under age 13), parent or other adult, MyChart proxy gives you access to the health information and tools you need to help take care of those who depend on you. Clinic, hospital, and urgent care records

There are two ways you can request copies of your medical records:

1. Submit an electronic request for records via our secure online portal.

*Please note: You may only request copies of your own record via the portal. If you are requesting records on another patient (including your minor children), then you will need to use option #2.

2. Complete a release form or send a signed and dated letter specifying what is to be released and to whom.

The release form is also available in SpanishRussian, and Vietnamese. We have included instructions on how to complete the release form. You can either fax, mail, or scan and email the form to us as noted below:

*Please Note: WA State law allows up to 15 business days to respond to a medical records request, but in most cases your request will be processed within 5 business days

Outpatient behavioral health and substance abuse records

There are two ways you can request copies of your medical records:

1. Complete a release form (also available in SpanishRussian, and Vietnamese) or send a signed and dated letter specifying what is to be released and to whom.

You can either fax, mail, or scan and email the form to us as noted below:

Health information management (medical records) locations

If you have additional questions, call the Health Information Department at 855-673-2673 Monday through Friday from 8am to 4:30pm. Closed on holidays.

Imaging records

Imaging films/CDs (X-rays, MRI scans, CT scans) are processed through the Imaging Records Management Department. You will have to complete our release form (also available in SpanishRussian, and Vietnamese) to get a copy of these records sent to you.

There is no cost for copies of your imaging records.
