{"id":32265,"date":"2024-02-28T07:00:36","date_gmt":"2024-02-28T07:00:36","guid":{"rendered":"https:\/\/updates-www.mccdn.io\/vitals\/?p=32265"},"modified":"2024-12-04T00:32:31","modified_gmt":"2024-12-04T00:32:31","slug":"tackling-ageism-in-health-care-part-2","status":"publish","type":"post","link":"https:\/\/updates-www.mccdn.io\/vitals\/tackling-ageism-in-health-care-part-2\/","title":{"rendered":"Tackling ageism in health care, part 2","gt_translate_keys":[{"key":"rendered","format":"text"}]},"content":{"rendered":"<p><em>At MultiCare, we are committed to working toward health equity \u2014 ensuring all people have access to quality health care regardless of age, race, ethnicity, gender identity, religion, sexual orientation, ability, socioeconomic status or location. <\/em><\/p>\n<p><em>But we also recognize the reality that longstanding biases and barriers embedded in the health care system, and our society, can prevent people from accessing the care they need. <\/em><\/p>\n<p><em>In this series \u2014 inspired in part by society\u2019s growing awareness that not everyone has the same chance to be healthy \u2014 we take a deeper look at the disparities that impact our patients and how we as health care providers and community members can reduce those disparities and improve the health and well-being of all.<\/em><\/p>\n<h2><strong>Ageism surrounds us<\/strong><\/h2>\n<p>Ten thousand people turn 65 every day in the U.S., according to <a href=\"https:\/\/www.aarpinternational.org\/initiatives\/aging-readiness-competitiveness-arc\/united-states\">AARP International<\/a>. By 2050, older adults will comprise more than 20 percent of the population.<\/p>\n<p>Yet despite the growing proportion of older adults in society, ageism \u2014 the stereotyping, prejudice, bias or discrimination toward people based on their age \u2014 is rampant.<\/p>\n<p>It shows up in <a href=\"https:\/\/www.forbes.com\/sites\/charlesrtaylor\/2022\/03\/11\/ageism-in-advertising-common-stereotypes-and-how-to-avoid-them\/?sh=1a65d5c8178c\">advertising<\/a>, in the <a href=\"https:\/\/generations.asaging.org\/ageism-media-insiders-perspective\">media we consume<\/a> and <a href=\"https:\/\/www.forbes.com\/sites\/nextavenue\/2021\/07\/30\/why-ageism-in-the-workplace-still-seems-to-be-okay\/?sh=77f9c71c668d\">in the workplace<\/a>. Ageism is also present in health care, leading to adverse outcomes, increased mortality, inability to access care and sometimes denial of care, <a href=\"https:\/\/www.nature.com\/articles\/s43587-020-00004-4#:~:text=Here%2C%20ageism%20is%20defined%20as%20age-related%20discrimination%2C%20including,care%20leading%20to%20adverse%20outcomes%20and%20increased%20mortality.\">according to an article in Nature Aging<\/a>.<\/p>\n<p>How do we take better care of older adults now \u2014 and lay the groundwork to take better care of our future selves?<\/p>\n<p>In the second of this two-part series, we further explore the effects of ageism and MultiCare Health System\u2019s efforts to combat it.<\/p>\n<h2><strong>Improving outcomes and clinical interactions for older adults<\/strong><\/h2>\n<p>Ageism in the health care setting affects older adults in a variety of ways, from how they are spoken to during a medical encounter to what diagnoses they receive.<\/p>\n<p>\u201cThroughout my career, I\u2019ve noticed this tendency to address older adults as \u2018sweetie\u2019 and \u2018honey,\u2019\u201d says Cathy Cooper, LICSW, a social worker specializing in <a href=\"https:\/\/updates-www.mccdn.io\/services\/palliative-care\/\">palliative care<\/a> at MultiCare Auburn Medical Center. \u201cWhile it\u2019s not meant to be disrespectful, it can be perceived that way. Instead, we should be asking people how they want to be addressed.\u201d<\/p>\n<p>Using terms of endearment in such a way can be part of <a href=\"https:\/\/www.aarp.org\/caregiving\/basics\/info-2017\/discouraging-elderspeak-from-caregivers-fd.html\">a style of speech known as elderspeak<\/a>, which \u2014 while common in health care settings \u2014 can be <a href=\"https:\/\/merrill.ku.edu\/elderspeak-it-helpful-or-just-baby-talk\">both patronizing and harmful<\/a> to patients, undermining their confidence in their own abilities.<\/p>\n<p>Sometimes ageism can lead to inaccurate or delayed diagnoses. Health care professionals may not always be aware of atypical disease presentations in older adults. <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0167494315300480?via%3Dihub\">One study found that at least one-third of seniors<\/a> who present at the emergency department with a serious infection do not have one of the hallmark symptoms: a fever.<\/p>\n<p>\u201cUrinary tract infections are common among older adults, yet they tend to cause confusion rather than some of the more typical symptoms we might see in a younger person,\u201d says <a href=\"https:\/\/updates-www.mccdn.io\/provider\/zeeba-mathews\/\">Zeeba Mathews, MD, a physician at MultiCare Good Samaritan Hospital who specializes in palliative care<\/a>. \u201cSometimes people mistakenly assume that a patient has long-standing dementia when really it\u2019s just an acute change because of an infection.\u201d<\/p>\n<p>While not every patient who presents at the emergency department (ED) with symptoms of delirium or confusion has dementia, EDs see a high proportion of patients with this diagnosis. <a href=\"https:\/\/jamanetwork.com\/journals\/jamaneurology\/article-abstract\/2807210?guestAccessKey=5dc18ef4-f483-49c7-8312-5bdeffdaabc1&amp;utm_source=For_The_Media&amp;utm_medium=referral&amp;utm_campaign=ftm_links&amp;utm_content=tfl&amp;utm_term=072423\">An article in JAMA Neurology<\/a> reports that approximately 25 percent of ED visits by those 65 and older are made by those with dementia or a related condition.<\/p>\n<p>\u201cHospitals are often not set up well to take care of these patients,\u201d Cooper says. \u201cResearch shows that even a short hospital stay can lead to a significant loss of functioning, and they may never regain those abilities even after they\u2019re discharged.\u201d<\/p>\n<p>To improve the care of patients with dementia and related conditions, Cooper has received a grant from MultiCare Foundations to launch a training initiative for hospital staff and volunteers at Auburn Medical Center.<\/p>\n<p>\u201cThe goal is to teach people best practices for interacting with patients who have dementia or are experiencing delirium,\u201d Cooper says. \u201cWe want to give people the hands-on skills they need for positive, safe, effective interactions.\u201d<\/p>\n<h2><strong>Ageism in research<\/strong><\/h2>\n<p>Beyond medical encounters, ageism can impact research \u2014 historically, older adults have been underrepresented in clinical trials. Trial protocols may set age limits for participants, or older adults may be indirectly excluded due to pre-existing chronic conditions or medications they\u2019re taking, <a href=\"https:\/\/academic.oup.com\/ageing\/article\/51\/5\/afab282\/6581615\">according to an Oxford University Press article<\/a>.<\/p>\n<p>Even if older adults are not outright excluded, the recruitment and retention of this population can be a challenge.<\/p>\n<p>\u201cSometimes potential participants are understandably hesitant to add yet another medical visit or medication to their schedule, which studies may require,\u201d says Jessica Horton, a clinical research supervisor at the MultiCare Institute for Research &amp; Innovation. \u201cMany studies also make use of technology like electronic diaries, which can be a deterrent for some older adults.\u201d<\/p>\n<p>Regardless of the reasons, exclusion of seniors in clinical trials is problematic \u2014potentially leading to gaps in knowledge about the safety, effectiveness and accurate dosing of new treatments for this age group.<\/p>\n<p>In 2023, the Research Institute initiated two vaccine-related studies specifically targeting older populations. One study, which is still enrolling those 65 and older, is testing a vaccine to prevent blood infections in people with a history of urinary tract infections. Another tested a vaccine for respiratory syncytial virus (RSV) in those 65 and older. The <a href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/72\/wr\/mm7240a2.htm?s_cid=mm7240a2_w\">Centers for Disease Control and Prevention<\/a> estimates that between 6,000-10,000 deaths occur each year among this age group due to RSV.<\/p>\n<p>\u201cA lot of our participants who are older adults are really excited to be a part of research,\u201d says Sarah Furth, another clinical research coordinator at the Research Institute. \u201cSimilar to other patient populations, they want to contribute to the greater good, improve care for themselves and for future generations in their family.\u201d<\/p>\n<blockquote style=\"font-size: 1rem;\">\n<h3>Suicide risk doesn\u2019t just affect the young<\/h3>\n<p>\u201cMen 75 and older have the highest risk of suicide out of any age group in the U.S. However, they are often left out of the conversation about suicide risk, and ageism plays a role in that.\u201d<br \/>\n\u2014 Julie Jensen, manager of Older Adult Services, MultiCare Behavioral Health Network<\/p><\/blockquote>\n<h2><strong>Supporting the mental health of seniors<\/strong><\/h2>\n<p>Psychological well-being is an important part of overall health across the lifespan. Yet mental illnesses such as anxiety and depression <a href=\"https:\/\/www.ncoa.org\/article\/mental-illness-is-not-a-normal-part-of-aging\">affect one in four older adults<\/a>, according to the National Council on Aging, and <a href=\"https:\/\/www.ncoa.org\/article\/how-to-improve-access-to-mental-health-and-substance-use-care-for-older-adults\">less than half of older adults <\/a>with mental health disorders receive treatment.<\/p>\n<p>\u201cAgeism can contribute to this treatment gap,\u201d says Julie Jensen, <a href=\"https:\/\/updates-www.mccdn.io\/services\/behavioral-health\/behavioral-health-programs-older-adults\/behavioral-health-programs-older-services\/\">manager of Older Adult Services for the MultiCare Behavioral Health Network<\/a>. \u201cSometimes people think that mental health conditions are a normal part of aging, but they aren\u2019t. This can lead to a lack of services and also discourages people from seeking help.\u201d<\/p>\n<p>MultiCare founded Older Adult Services 30 years ago when Washington state identified seniors as one of the most underserved in terms of access to mental health care, according to Jensen.<\/p>\n<p>Today, Older Adult Services \u2014 which serves more than 500 patients a month \u2014 remains one of the few community mental health programs available in the Pacific Northwest that focuses solely on seniors.<\/p>\n<p>Based in Puyallup, Older Adult Services delivers short-term outpatient mental health care to adults 60 and over who are dealing with issues such as grief and loss, age-related cognitive conditions, substance use, depression, suicidality and more.<\/p>\n<p>\u201cMen 75 and older have the highest risk of suicide out of any age group,\u201d Jensen says. \u201cHowever, they are often left out of the conversation about suicide risk, and ageism plays a role in that.\u201d<\/p>\n<p>Older Adult Services also works closely with the <a href=\"https:\/\/updates-www.mccdn.io\/services\/behavioral-health\/behavioral-health-programs-older-adults\/behavioral-health-programs-older-psychiatry\/\">Geriatric Inpatient Psychiatry Unit at Auburn Medical Center<\/a>, which specializes in treating older adults with dementia and disorders such as schizophrenia, depression and anxiety.<\/p>\n<p>\u201cThere\u2019s a tendency for older adults to suffer in silence when it comes to mental health concerns, but nobody should have to do that,\u201d Jensen says. \u201cWe focus on providing counseling and giving people the tools they need to get better.\u201d<\/p>\n","protected":false,"gt_translate_keys":[{"key":"rendered","format":"html"}]},"excerpt":{"rendered":"<p>At MultiCare, we are committed to working toward health equity \u2014 ensuring all people have access to quality health care regardless of age, race, ethnicity, gender identity, religion, sexual orientation, ability, socioeconomic status or location.
