{"id":36533,"date":"2025-02-21T07:00:35","date_gmt":"2025-02-21T07:00:35","guid":{"rendered":"https:\/\/updates-www.mccdn.io\/vitals\/?p=36533"},"modified":"2025-02-28T22:44:46","modified_gmt":"2025-02-28T22:44:46","slug":"diagnosed-at-34-nurse-beats-rectal-cancer","status":"publish","type":"post","link":"https:\/\/updates-www.mccdn.io\/vitals\/diagnosed-at-34-nurse-beats-rectal-cancer\/","title":{"rendered":"Diagnosed at 34, nurse beats rectal cancer","gt_translate_keys":[{"key":"rendered","format":"text"}]},"content":{"rendered":"<p>A change in bowel habits is something you should never ignore \u2014 no matter your age.<\/p>\n<p>Katie Dutton, RN, was only 33 when she noticed blood in her stool.<\/p>\n<p>\u201cIt was the summer of 2022. I was a travel nurse at the time,\u201d Dutton recalls. \u201cI didn\u2019t have a primary care provider in place, so I went to a few different doctors. They figured I had hemorrhoids that were acting up due to constipation and recommended I eat more fiber.\u201d<\/p>\n<p>Adding fiber to her diet didn\u2019t alleviate her symptoms.<\/p>\n<p>The following winter, Dutton started working at MultiCare. No longer traveling around the state for work, she established care with MultiCare family physician <a href=\"https:\/\/updates-www.mccdn.io\/provider\/marissa-robinson\/\">Marissa Robinson, MD<\/a>.<\/p>\n<p>\u201cDr. Robinson immediately took my symptoms seriously,\u201d she says. \u201cShe told me I needed a colonoscopy and tested me for Crohn\u2019s and other autoimmune diseases.\u201d<\/p>\n<p>The tests for those conditions were negative. The colonoscopy, however, revealed a large polyp. Polyps are typically removed during colonoscopies, as they are at risk for becoming cancerous, but this one was too large to remove during the procedure. A biopsy was taken instead, and it came back negative for cancer.<\/p>\n<h2><strong>Biopsies continue to show negative results<\/strong><\/h2>\n<p>That\u2019s when Dutton was referred to MultiCare colorectal surgeon <a href=\"https:\/\/updates-www.mccdn.io\/provider\/laila-rashidi\/\">Laila Rashidi, MD<\/a>.<\/p>\n<p>Dr. Rashidi ordered a CT scan and MRI. A CT scan will show if there\u2019s cancer anywhere else in the body; the MRI looks at surrounding lymph nodes and tumor size.<\/p>\n<p>\u201cThe CT scan didn\u2019t show any distant metastatic disease \u2014 the tumor was local to the rectum,\u201d Dr. Rashidi notes. \u201cThe MRI didn\u2019t indicate that it had spread to surrounding tissues, but a lymph node in the area appeared to be involved.\u201d<\/p>\n<p>Although a second biopsy of the mass still didn\u2019t show cancer cells, Dr. Rashidi and Dutton\u2019s medical oncologist, <a href=\"https:\/\/updates-www.mccdn.io\/provider\/jennifer-slim\/\">Jennifer Slim, DO<\/a>, continued to suspect cancer.<\/p>\n<p>\u201cDr. Slim said it looked like cancer on the MRI, but the biopsies kept coming back negative,\u201d Dutton says.<\/p>\n<p>The standard protocol for rectal cancer is chemotherapy and radiation, then surgery. But because there was no evidence of cancer, she was scheduled for surgery to remove the mass and some of the surrounding tissue so it didn\u2019t have the chance to develop into cancer.<\/p>\n<h2><strong>A change of plan<\/strong><\/h2>\n<p>Accredited by the National Accreditation Program for Rectal Cancer, MultiCare Cancer Institute takes a multidisciplinary approach to treatment. Medical, surgical and radiation oncology, pathology and radiology all come together to discuss treatment protocols and patient progress at weekly tumor board meetings.<\/p>\n<p>\u201cTumor board met the morning of Katie\u2019s surgery,\u201d Dr. Rashidi recalls. \u201cWe spent 60 percent of our time on her case that morning. There was a concern the lymph node in question may be related to a cancer, which meant her surgery needed to be canceled.\u201d<\/p>\n<p>A concerning lymph node typically indicates cancer has spread from a nearby tumor, so another biopsy of the polyp was needed. If cancer was found, Dutton would first need chemotherapy. Surgery wouldn\u2019t be performed until after chemo treatment.<\/p>\n<p>\u201cI was all prepped for surgery,\u201d Dutton explains. \u201cI took the pre-surgery antibiotics, I did the bowel prep, I took the disinfectant shower. I was just about to leave the house at 8am, when I got a call from Dr. Rashidi.\u201d<\/p>\n<p>Dr. Rashidi called Dutton right after the meeting to let her know the plan changed.<\/p>\n<p>\u201cI told her not to come to the hospital \u2014 we weren\u2019t going to do surgery,\u201d Dr. Rashidi says. \u201cI felt so bad when I called her because she was all prepped for surgery. She was so understanding and said she trusted us.\u201d<\/p>\n<p>Dr. Slim says that Dutton\u2019s case demonstrates how important it is to have a cohesive team.<\/p>\n<p>\u201cHaving that multidisciplinary approach allowed the surgeon, oncologist, radiologist and radiation oncologist to get together and agree how to treat her safely and effectively,\u201d she adds. \u201cI couldn\u2019t imagine if we could have treated her in such a timely manner if we didn\u2019t have the tumor board.\u201d<\/p>\n<h2><strong>Persistence of care team leads to cancer diagnosis<\/strong><\/h2>\n<p>Upon recommendation of the tumor board, Dutton had a third biopsy of the polyp.<\/p>\n<p>\u201cI\u2019ll never forget \u2014 it was Black Friday when Dr. Rashidi called me to tell me the results,\u201d Dutton recalls. \u201cCancer in the mass was finally found. I started chemo within a week.\u201d<\/p>\n<p>She went through eight rounds of chemo \u2014 every two weeks for four months. Her boyfriend Drew was by her side, accompanying her to every appointment.<\/p>\n<div id=\"attachment_36537\" style=\"width: 310px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-36537\" class=\"size-medium wp-image-36537\" src=\"https:\/\/updates-www.mccdn.io\/vitals\/wp-content\/uploads\/sites\/15\/2025\/02\/Katie-and-Drew-300x225.jpg\" alt=\"Woman and man smiling at camera during a medical treatment. \" width=\"300\" height=\"225\" srcset=\"https:\/\/updates-www.mccdn.io\/vitals\/wp-content\/uploads\/sites\/15\/2025\/02\/Katie-and-Drew-300x225.jpg 300w, https:\/\/updates-www.mccdn.io\/vitals\/wp-content\/uploads\/sites\/15\/2025\/02\/Katie-and-Drew-1024x769.jpg 1024w, https:\/\/updates-www.mccdn.io\/vitals\/wp-content\/uploads\/sites\/15\/2025\/02\/Katie-and-Drew-768x577.jpg 768w, https:\/\/updates-www.mccdn.io\/vitals\/wp-content\/uploads\/sites\/15\/2025\/02\/Katie-and-Drew-1536x1154.jpg 1536w, https:\/\/updates-www.mccdn.io\/vitals\/wp-content\/uploads\/sites\/15\/2025\/02\/Katie-and-Drew.jpg 1599w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><p id=\"caption-attachment-36537\" class=\"wp-caption-text\">Dutton with boyfriend Drew at a chemo treatment<\/p><\/div>\n<p>\u201cKatie\u2019s partner was very supportive, understanding and inquisitive throughout her treatment,\u201d Dr. Rashidi adds. \u201cThey both were truly teammates with us.\u201d<\/p>\n<p>In addition to Drew, Dutton received tremendous support from family and friends.<\/p>\n<p>\u201cMy brother, who lives in Seattle, would go hiking with me every other weekend, on the weeks I didn\u2019t have chemo,\u201d Dutton says. \u201cIt felt good to get outside and do something active.\u201d<\/p>\n<h2><strong>New trial alters course of treatment<\/strong><\/h2>\n<p>Chemotherapy and radiation before surgery is standard care for rectal cancer, but a study called the PROSPECT trial came out around the time of Dutton\u2019s diagnosis that showed in many cases radiation can be avoided.<\/p>\n<p>For Dutton, not having to do radiation of the pelvic area meant she could preserve her fertility.<\/p>\n<p>\u201cThe change in plan for my treatment ended up being a good thing,\u201d Dutton explains. \u201cRadiation to the pelvis puts you in premature menopause, and I want to be able to have kids in the future.\u201d<\/p>\n<div id=\"attachment_36539\" style=\"width: 235px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-36539\" class=\"size-medium wp-image-36539\" src=\"https:\/\/updates-www.mccdn.io\/vitals\/wp-content\/uploads\/sites\/15\/2025\/02\/hiking-pic-225x300.png\" alt=\"two women and one man on hike \" width=\"225\" height=\"300\" srcset=\"https:\/\/updates-www.mccdn.io\/vitals\/wp-content\/uploads\/sites\/15\/2025\/02\/hiking-pic-225x300.png 225w, https:\/\/updates-www.mccdn.io\/vitals\/wp-content\/uploads\/sites\/15\/2025\/02\/hiking-pic.png 542w\" sizes=\"auto, (max-width: 225px) 100vw, 225px\" \/><p id=\"caption-attachment-36539\" class=\"wp-caption-text\">Dutton on a hike with her friend and brother between chemo treatments<\/p><\/div>\n<p>\u201cIt was advantageous to be able to offer her and her family a new treatment that allows them to maintain their goal as a family,\u201d Dr. Slim says.<\/p>\n<p>After her chemo treatment, Dutton needed one final surgery \u2014 a minimally invasive robotic ultra-low anterior resection \u2014 to remove most of her rectum and surrounding lymph nodes.<\/p>\n<p>\u201cKatie\u2019s tumor was low, but we were able to preserve her sphincter muscle,\u201d Dr. Rashidi explains. \u201cShe started walking on her own the same day and was discharged from the hospital the next day.\u201d<\/p>\n<p>The lymph node in question was tested and found to be cancerous, which meant Dutton needed two more months of chemo.<\/p>\n<p>Since then, Dutton\u2019s had a scan every three months. Tiny nodules were found on her lungs, but she says they aren\u2019t growing and will continue to be monitored.
