{"id":39257,"date":"2026-04-16T07:00:28","date_gmt":"2026-04-16T07:00:28","guid":{"rendered":"https:\/\/updates-www.mccdn.io\/vitals\/?p=39257"},"modified":"2026-04-13T20:44:13","modified_gmt":"2026-04-13T20:44:13","slug":"pcos-differences-between-teens-and-adults","status":"publish","type":"post","link":"https:\/\/updates-www.mccdn.io\/vitals\/pcos-differences-between-teens-and-adults\/","title":{"rendered":"PCOS: Differences between teens and adults","gt_translate_keys":[{"key":"rendered","format":"text"}]},"content":{"rendered":"<p>Polycystic ovarian syndrome (PCOS),\u202fa\u202fhormonal condition\u202fthat affects\u202fan estimated\u202fone in 10 women,\u202fhas a wide array of symptoms\u00a0and\u00a0is often\u00a0underdiagnosed.<\/p>\n<p>The variety of symptoms\u202fmeans that not everyone experiences PCOS in the same way.\u202fDiagnosis and treatment\u202fapproaches\u00a0also\u00a0differ\u202ffor teens\u202fand\u202fadults,\u202fwhich can make it more confusing.\u202fPlus, social media\u202fadds to the confusion,\u202ftouting\u202fmiracle \u201ccures,\u201d\u202fspecial\u202fdiets and supplements.<\/p>\n<p>But understanding what PCOS\u202fis and how treatment is approached\u00a0\u2014\u00a0especially\u202fin\u202fadolescents\u202fversus\u202fadults\u00a0\u2014\u202fcan help clear up misconceptions.<\/p>\n<h2>What is PCOS?<\/h2>\n<p>Polycystic ovarian syndrome\u00a0happens when\u202fthe ovaries produce\u202fhigher-than-normal\u202flevels\u202fof a particular type of\u202fhormone\u202fcalled androgens\u202f(testosterone\u00a0is a type of androgen).\u202fThese elevated levels can interfere with\u202fregular\u202fovulation.<\/p>\n<p>Common signs of PCOS include:<\/p>\n<ul>\n<li>Irregular periods\u202f(skipping periods, no periods at all\u00a0or periods that are too frequent)<\/li>\n<li>Abnormal bleeding<\/li>\n<li>Excess\u202fhair on the face or other parts of the body, called hirsutism<\/li>\n<li>Acne<\/li>\n<li>Weight gain\u202for\u202ftrouble losing weight<\/li>\n<\/ul>\n<h2>How\u202fPCOS\u202fis\u202fdiagnosed<\/h2>\n<p><a href=\"https:\/\/updates-www.mccdn.io\/provider\/kara-ayers\/\">Kara\u202fAyers, ARNP<\/a>, with\u202fMultiCare Women&#8217;s Health\u202fin\u202fTacoma,\u202foften uses\u202fthe Rotterdam criteria\u202fto make a\u202fdiagnosis\u00a0of PCOS in adults.\u202fThis method of diagnosis\u202frequires the presence of at least two\u202fof the three\u202ffollowing\u202fsymptoms:\u202firregular or absent periods, excess androgen\u202flevels\u202f(hyperandrogenism)\u00a0or\u00a0polycystic ovaries\u00a0on ultrasound.<\/p>\n<p>\u201cIn my practice, the\u00a0symptom\u00a0that\u00a0brings\u00a0people in the most\u00a0is\u00a0irregular periods,\u201d Ayers says.\u202f\u201cI also look for\u202fsigns of excessive metabolic dysfunction\/insulin resistance\u202f\u2014\u202fexcessive weight gain, difficulty losing weight.\u201d<\/p>\n<p>Ayers says she also\u202flooks\u202ffor acne\u202fthat&#8217;s\u202fpersisted\u202finto\u202fadulthood\u202fand is difficult to treat,\u00a0or abnormal hair growth on the jawline\u00a0or in typical male patterns.<\/p>\n<p>Although the name of the condition refers to\u202fcysts\u202fon the ovaries,\u202ftheir presence\u202fis\u202fnot\u202frequired\u202ffor diagnosis.<\/p>\n<p>When it comes to diagnosing PCOS in\u00a0teens,\u202fthe approach is\u202fslightly\u202fdifferent.\u00a0Rather than\u00a0the Rotterdam criteria,\u00a0the\u00a0National\u00a0Institutes\u00a0of Health\u00a0(NIH)\u00a0criteria\u00a0is\u00a0used\u00a0for adolescents. They overlap,\u00a0but NIH criteria only\u00a0uses\u00a0menstrual irregularity and\u00a0hyperandrogenism and excludes\u00a0polycystic ovaries\u00a0on ultrasound.<\/p>\n<p>\u201cWe don\u2019t use ultrasound as a diagnostic tool for teens, as adolescent ovaries tend to look polycystic anyway,\u201d says\u202f<a href=\"https:\/\/www.marybridge.org\/provider\/carri-holton\/\">Carri\u202fHolton, ARNP<\/a>, with Mary Bridge Children\u2019s Pediatric &amp; Adolescent Gynecology.\u202f\u201cFor an adolescent with PCOS, it\u00a0can present as irregular periods, absent periods\u00a0or never starting their period at all.\u201d<\/p>\n<p>In addition to period patterns, lab work showing elevated testosterone\u202fand visible signs,\u00a0like\u202fpersistent acne\u202for excess body\u202fhair,\u202fare other\u202fsymptoms that can point to\u202fa PCOS\u202fdiagnosis.<\/p>\n<h2>Treating\u202fPCOS<\/h2>\n<p>Because PCOS presents so\u00a0differently\u00a0person to person, treatment is not one-size-fits-all.\u202fThe goal is to manage symptoms and protect long-term health.<\/p>\n<p>\u201cYou can get 100 patients with\u202fPCOS,\u202fand they are all\u202fa little different,\u202fso\u202ftheir treatment is\u202ftailored\u202fto their\u202fsymptoms,\u201d\u202fAyers says.<\/p>\n<p>A common treatment\u202ffor both adults and teens\u202fis\u202fbirth control, which can help regulate periods,\u202fprotect the lining of the uterus,\u202freduce acne and control excessive hair growth by lowering testosterone levels.<\/p>\n<p>And for those trying to conceive, metformin \u2014 a medication used to regulate insulin \u2014 can help improve\u00a0menstrual\u00a0cycles,\u00a0which\u00a0can\u00a0in turn\u00a0improve\u00a0ovulation and fertility.<\/p>\n<p>Spironolactone, an androgen blocker, is sometimes used to address symptoms like acne and hair growth.<\/p>\n<p>\u201cBecause a teen\u2019s\u00a0primary\u00a0concern isn\u2019t about their fertility, our treatment focus is\u202fon lifestyle\u202fand\u202fsymptom management,\u202fsuch as\u202fmenstrual regulation,\u00a0acne\u202ftreatments\u00a0and\u00a0nutrition counseling,\u201d\u202fHolton says.\u202f\u201cWe\u202fwant to set\u202fthem up for long-term health.\u201d<\/p>\n<h2>Weight management for PCOS<\/h2>\n<p>An effective treatment for both teens and adults\u202fis\u202fweight management. Even a modest amount of weight loss\u202f\u2014\u202fabout 5 percent of body\u00a0weight\u202f\u2014\u202fcan\u202fregulate\u202fcycles,\u202fprotect\u202ffertility\u202fand\u202freduce the\u202feffects of\u202fandrogens.<\/p>\n<p>\u201cAnd it\u2019s not\u202fjust for symptom relief, but to protect from long-term complications from the condition,\u201d Ayers says.<\/p>\n<p>Weight loss can lower the risk of developing conditions like\u00a0Type 2 diabetes, heart\u202fdisease\u202fand sleep apnea.<\/p>\n<p>\u201cThe development of\u202fGLP-1 medications has been an exciting step\u202ffor\u202fPCOS patients to get the\u202fassistance they need to lose the weight that is so\u00a0easily\u202fgained and so hard to lose,\u201d\u00a0she adds.<\/p>\n<h2>Fertility and PCOS<\/h2>\n<p>Fertility\u202fis one of\u202fthe most\u202fcommon concerns adults may have after a PCOS diagnosis.\u202fThe good news is that birth rates for those with PCOS\u202fare\u202fsimilar to\u00a0those without it.\u202fThe challenge is often\u202ftiming:\u202fIt may take longer to conceive than the average person\u202fbecause of irregular ovulation\u00a0and may require more fertility\u00a0assistance.<\/p>\n<p>When the time comes to\u202ftry\u202ffor\u202fa baby, there are effective options. Medications like letrozole can help trigger ovulation.<\/p>\n<p>\u201cMy whole goal is to just try to keep\u00a0my\u00a0patients\u00a0as healthy as I can until they&#8217;re ready to get pregnant,\u201d\u202fAyers adds.<\/p>\n<h2>\u2018Natural\u2019 alternatives to medication<\/h2>\n<p>\u201cThere is\u202fso much misunderstanding\u202fwith\u202fPCOS,\u201d\u00a0Holton says.\u202f\u201cThere\u2019s\u202fa lot\u202fon social media about it,\u202fand\u202fpeople\u202fdon\u2019t\u202funderstand what it is.\u201d<\/p>\n<p>With PCOS\u202fbeing so\u202fwidely discussed online,\u202fit\u2019s\u202fcommon for patients to ask about natural alternatives to medication.\u202fAyers says patients\u202fwill\u202fask about taking supplements versus hormones\u202for if special diets work.<\/p>\n<p>\u201cNatural\u202fdoesn\u2019t\u202falways\u202fmean\u202fsafe\u202f\u2014\u202fwe\u202fdon\u2019t\u202fhave good studies on\u202f(natural remedies)\u00a0or know how they affect high androgen or insulin levels,\u201d\u202fshe says.\u202f\u201cPCOS\u202fis a hormonal disorder,\u202fso we often use hormones to treat it.\u201d<\/p>\n<p>That\u2019s\u202fwhy hormone\u202fmedications and\u202flifestyle\u202fmodifications like weight management are\u202fcommon and effective\u202ftreatments.<\/p>\n<p>\u201cIf you want to be 100\u202fpercent\u202fnatural, it&#8217;s going to be lifestyle modification,\u201d Ayers adds.\u202f\u201cThere&#8217;s no\u202fspecific,\u202fproven diet\u202ffor\u202fPCOS.\u202fIt&#8217;s\u202fjust going to be calorie restriction\u202fand\u202fportion\u202fcontrol.\u201d<\/p>\n<h2>Managing PCOS in the long term<\/h2>\n<p>Whether in adolescence or adulthood, managing PCOS requires a long-term commitment to health.\u202fThe most important part of managing PCOS is finding the right treatment plan that works for them.<\/p>\n<p>\u201cAs a gynecologic surgeon, if I suspect a patient has PCOS, I try to reiterate that this is a condition that is managed medically and does not require surgery,&#8221;\u00a0adds\u00a0<a href=\"https:\/\/updates-www.mccdn.io\/provider\/molly-dorroh\/\">Molly Dorroh, DO<\/a>, with MultiCare Women&#8217;s Health\u202fin\u202fTacoma. &#8220;PCOS often requires a collaborative team of providers to effectively manage the wide variety of symptoms.\u201d<\/p>\n","protected":false,"gt_translate_keys":[{"key":"rendered","format":"html"}]},"excerpt":{"rendered":"<p>Polycystic ovarian syndrome (PCOS),\u202fa\u202fhormonal condition\u202fthat affects\u202fan estimated\u202fone in 10 women,\u202fhas a wide array of symptoms\u00a0and\u00a0is often\u00a0underdiagnosed.
