Patient-Centered Medical Home

MultiCare Health System remains financially stable while continuing to expand services and regional impact. Tacoma Family Medicine maintains designation as a NCQA Level 3 Patient-Centered Medical Home (PCMH). While providing care in our PCMH, residents learn from a multi-disciplinary staff of professionals, including an on-site clinical pharmacist, a social worker, a behavioral scientist/psychologist, a psychiatrist, a pediatrician, a certified nurse-midwife, behavioral health counselors, a population health RN, and a chronic disease care coordinator. Our clinic has had an integrated EMR (Epic) for over 20 years, and we have been providing video visits for the past 3 years (work that has expanded further with the current COVID-19 pandemic).

Our Patient Population

Pierce County is one of the most ethnically diverse counties in Washington State. TFM’s clinic is located in Tacoma’s urban center, serving a patient panel that reflects the diversity of our area. Approximately 70% of our patients are on Medicaid or Medicare. TFM has an ongoing project to identify and address social determinants of health for our patients. Our family medicine inpatient service cares for our clinic patients as well as patients from the Puyallup Tribal Health Authority, adding unique and rich cross-cultural patient perspectives. Our faculty and residents take pride in working as a team to provide the highest standard of medicine to our local community.
