PGY2 Ambulatory Care Pharmacy residency

MultiCare’s PGY2 Ambulatory Care Pharmacy Residency develops competent clinical practitioners who are ready to provide evidence-based, patient-centered care to a diverse patient population. Ambulatory care clinical pharmacists provide direct patient care under collaborative drug therapy agreements as credentialed providers, working alongside the interdisciplinary healthcare team. Resident rotations are primarily located in Tacoma and Puyallup, Washington.

PGY2 ambulatory care pharmacy residents at MultiCare have a variety of learning opportunities and excellent support, including a formal mentorship program. Pharmacy residents are an integral part of MultiCare’s family medicine residency programs at MultiCare Tacoma Family Medicine and MultiCare East Pierce Family Medicine, practicing longitudinally in the family medicine teaching clinics where they gain experience as clinical instructors for medical residents and preceptors for pharmacy residents and students. The program emphasizes teaching and scholarship, with a yearlong preceptor development program and opportunities to precept pharmacy learners. Residents gain broad experiences in many areas of ambulatory care pharmacy, with a focus on medical management of chronic diseases. Graduates are eligible to sit for board certification exams in ambulatory care pharmacy, advanced diabetes management and diabetes care and education.

Curriculum

The residency program prepares graduates to be skillful clinicians in a variety of ambulatory care practices. The curriculum can be tailored to each resident by offering a range of electives, allowing them to gain experience in many settings or focus on a particular area of interest.

Residents primarily practice at our Family Medicine Residency clinics, where they are fully integrated into patient care and provide interdisciplinary medical education. Clinical pharmacists are consulted for a wide variety of needs, and they see patients directly for visits such as anticoagulation, asthma and COPD, diabetes, heart failure, hypertension, hyperlipidemia, polypharmacy, and weight management among others.

Residents complete the following required longitudinal learning experiences:

  • Family medicine residency
  • Primary care clinic
  • Managed care – population health
  • Academics – teaching and precepting
  • Research – residency project

Elective learning experiences comprise sixteen weeks, and residents may collaborate with preceptors to develop new electives based on their interests:

  • Addiction Medicine
  • Diabetes
  • Cardiology in Yakima
  • Hospice / Palliative Care
  • Geriatrics
  • Oncology
  • Psychiatry
  • Rheumatology / Specialty Pharmacy
  • Rural Family Medicine in Omak

Staffing commitment

Residents teach a group diabetes education class one weekday evening per month, a part of the diabetes education class series which is repeated monthly. It is an excellent opportunity for residents to gain experience in facilitating groups and counseling patients, and can be counted toward patient care hours for CDCES or BC-ADM exams. There are no weekend staffing commitments.

Leanna Davis, PharmD, BCACP, CDCES

Faculty Pharmacist, PGY-2 Ambulatory Care Residency Director

About Me: Hello, my name is Leanna Davis and I am the Faculty Pharmacist at Tacoma Family Medicine residency program. I have been with TFM since 2022 and with MultiCare since 2012. My role is to help educate and train medical residents, and I also precept PGY1 and PGY2 pharmacy residents as they see ambulatory care patients for chronic disease management.

Professional Background: I earned my PharmD from WSU in 2010 and completed my PGY1 at St. Joseph Medical Center in Tacoma. I am board-certified in ambulatory care, and my professional interests include diabetes management, substance use disorder, gender affirming care, and preceptor development. I am passionate about interdisciplinary care and teaching, especially the role of experiential learning and postgraduate training.

Personal Interests: Outside of work, I enjoy spending time with my family, especially outdoors in our beautiful state!

Christy Weiland, PharmD, BCPS, BC-ADM

Faculty Pharmacist, PGY-2 Ambulatory Care Residency Coordinator

About me: Hello, my name is Christy Weiland, and I am a Faculty Pharmacist at East Pierce Family Medicine residency program within MultiCare. I have been here since 2020. My role is to teach and provide guidance to family medicine physicians. I practice both in the inpatient setting with the family medicine team and at the clinic. Some of the activities I am involved in are curriculum development, scholarship work and chronic disease management.

Professional Background: I earned my PharmD from University of Washington (go Dawgs!) in 2007 and completed my PGY-1 at St. Louis College of Pharmacy. I am board-certified as a pharmacotherapy specialist and Advanced Diabetes Management. Throughout my career, I’ve enjoyed the opportunity to work in a faculty position within family medicine and emergency medicine, dedicating time to scholarship work as well as supporting policy change at a state level.

Personal Interests: Outside of work, I enjoy spending time with my family, hiking all over the PNW as well as boating.

Amanpreet (Ammy) Bhathal completed her PharmD at Roseman University of Health Sciences in Henderson, NV followed by her PGY1 residency with PeaceHealth St. John Medical Center in Longview, WA. Prior to pharmacy school, Ammy grew up in Everett, WA and received her undergraduate degree from the University of Washington followed by her MPH from A.T. Still University. Her longitudinal project this year is Implementing a Kidney Care Pathway to Improve Identification and Management in Primary Care. MultiCare’s residency program interested Ammy due to the variety of experiences offered such as primary care, specialty pharmacy, and diabetes education which allows her to explore multiple career pathways within ambulatory care. In her spare time, she enjoys crafting and arts, binge-watching shows, and spending time with her friends and family, especially her husky, Heera!

Alisa Johnson is originally from Las Vegas, NV where she graduated from Roseman University with her PharmD. She completed her PGY1 residency with Virginia Mason in Seattle, WA. Her career interests include diabetes management and teaching. Her longitudinal research project is Continuous Glucose Monitor Monitoring in Maternal Fetal Medicine Patients with Gestational Diabetes Mellitus. Alisa enjoys spending time with her family, hiking outdoors, and shopping.


Lynn Luu completed her PGY2 ambulatory care residency training with MultiCare in 2024. She works as a clinical ambulatory care pharmacist at MultiCare’s pharmacotherapy clinics, managing medications for patients with diabetes and other chronic conditions.


Taylor Sims completed her PGY2 ambulatory care residency training with MultiCare in 2023. She is a board-certified ambulatory care pharmacist with MultiCare Hospice and is a preceptor for the PGY2 ambulatory care pharmacy residency. In her role as a clinical pharmacist, Taylor works with a multidisciplinary team to provide education, recommendations, and medication management for end-of-life symptoms.


Amanda Brookhaus completed her PGY2 ambulatory care residency training with MultiCare in 2022. She works for Kaiser Permanente as a board certified ambulatory care pharmacist. Her position includes managing medications for chronic diseases, completing in depth medication reviews and reconciliations, and providing consults to providers.

Jalena Bown, PharmD

Hospice Clinical Pharmacist

About me: Hello, my name is Jalena and I work in Hospice specializing in palliative care in a variety of ambulatory settings. I have been with MultiCare since 2016, and my role in hospice includes a wide variety of pharmacy related duties, including medication prescribing and dose adjustments, hospice interdisciplinary team meetings, pharmacy clinical pearls, formulary/clinical med reviews, Transitions of care (TOC) coordination, deprescribing, pain management, and lots of patient and caregiver education.

Professional background: I earned my Doctor of Pharmacy (PharmD) from Washington State University in 2014 and completed my PGY-1 Residency at Yakima Memorial (now Multicare) Hospital. I am licensed with the DEA and have certificates in pain management/palliative care and my professional interests include system process improvements (esp TOC), pharmacokinetics/dynamics and how it changes with age and as we approach end of life. Throughout my career, I’ve enjoyed the opportunity to work with many diverse healthcare clinicians and see the scope of impact we all can make to improve patient outcomes, the pharmacologic and non-pharmacologic. It has been rewarding to provide experiences to students/residents on hospice care and how the goals and logistics of caring for people as they approach end of life change, at times significantly.

Personal interests: Outside of work, I enjoy nature photography and spending time outdoors. Living life to the fullest and playing Magic The Gathering online with other amazing and cool people!

Khushi Chahal, PharmD, BCACP

Geriatrics Clinical Pharmacist

About me: Hello, my name is Khushi Chahal, and I am a Clinical Pharmacist with PNW PACE.  I have been with MultiCare since 2024, and my role here includes providing direct patient care while collaborating with interdisciplinary teams.

Professional background: I earned my Doctor of Pharmacy (PharmD) from University of California, San Diego in 2017 and completed my PGY-1 Residency in Pharmacy Practice and PGY-2 Residency in Ambulatory Care at Kaweah Health. I am board-certified in Ambulatory Care Pharmacy (BCACP), and my professional interests include diabetes management and transitions of care. Throughout my career, I’ve enjoyed the opportunity to work with underserved populations and mentor learners.

Personal interests: Outside of work, I enjoy hiking, listening to audiobooks/podcasts, and spending time with family.

Lead Pharmacist/Gig Harbor Medication Management Clinic

About me: Hello, my name is Jeannette am the Lead Pharmacist in the Gig Harbor Medication Management Clinic. I have been with MultiCare since 2018, and my role here includes providing direct patient care and ensuring that the clinic meets all regulatory requirements.

Professional background: I earned my Doctor of Pharmacy (PharmD) from the University of Washington in 2018 and completed my PGY-1 at MultiCare- Tacoma General. My professional interests include Ambulatory Care. Throughout my career, I’ve enjoyed the opportunity to build strong patient relationships while expanding this clinic.

Johnny Chang, PharmD, BCPS, CSP

Specialty Pharmacy Supervisor

About me: Hello. My name is Johnny, and I am the Specialty Pharmacy Supervisor. I have been a dedicated member of the Multicare team since 2023, supporting the rheumatology clinics and precepting residents and pharmacy students. My role also includes supporting specialty pharmacy services and providing direct patient care in the rheumatology clinics.

Professional background: I earned my Doctor of Pharmacy from the University of Michigan in 2018 and completed my PGY-1 at Novant Health Specialty Pharmacy. I am board-certified in pharmacotherapy and a Certified Specialty Pharmacist. My professional interests include specialty pharmacy, teaching, and promoting the pharmacy profession. Throughout my career, I’ve enjoyed the opportunity to expand my past residency program, create new clinical pharmacist roles, and spread the awareness of Specialty Pharmacy locally and nationally.

JT Lew, PharmD, BCPS

Managed Care Pharmacist, MultiCare Connected Care

About me: Hello, my name is JT and I am a Managed Care Pharmacist on the MultiCare Connected Care team, along with Michael Lee, PharmD. I have been with MultiCare since 2021, and my role here includes collaborating with payors, who have risk-based agreements with the health system, on drug utilization management, clinical program, and other population health or managed care-related opportunities to reduce the total cost of care for patients, the health system, and payors.

Professional background: I earned my Doctor of Pharmacy (PharmD) from the University of Washington in 2020 and completed my PGY-1 Residency in Managed Care at Kaiser Permanente Washington. I am board-certified in Pharmacotherapy, and my professional interests include, population health management, preventive care, and specialty pharmacy. Throughout my career, I’ve enjoyed the opportunity to volunteer with professional associations, such as the Northwest Affiliate of the Academy of Managed Care Pharmacy (NW AMCP) and Toastmasters International , mentor pharmacy students and residents, and explore ways to expand access to value-based care.

Personal interests: Outside of work, I enjoy watching live sporting events on TV (and attending them in-person when possible), trying new restaurants (especially BBQ), traveling, and trying new outdoor activities.

Taylor Sims, PharmD, BCACP

Hospice Clinical Pharmacist

About me: Hello, my name is Taylor and I am a Clinical Pharmacist in Hospice. I have been with MultiCare since 2021, and my role in Hospice includes operating under a CDTA and providing education, recommendations, and medication management for end-of-life symptoms.

Professional background: I earned my Doctor of Pharmacy (PharmD) from University of Washington in 2021 and completed my PGY1 at Tacoma General and PGY2-ambulatory care at MultiCare. I am board-certified in ambulatory care pharmacy, and my professional interests include end-of-life symptom management, pain management, and medication safety. Throughout my career, I’ve enjoyed the opportunity to work within an amazing multidisciplinary team and precept pharmacy learners.

Personal interests: Outside of work, I enjoy reading, crocheting, and hanging out with my 4 cats.

Eligible candidates will be a graduate (prior or anticipated) of an ACPE-accredited college of pharmacy or Foreign Pharmacy Graduate Equivalency Committee (FPGEC) certificate, and is licensed or eligible for licensure in Washington state.
