Medical Records

We offer several options for you to access your health information, including via the MyChart patient portal.

We’re committed to protecting your medical information by keeping it secure and confidential. There are a few ways to obtain access and get copies of your medical records, depending on where you received care. Learn more about how to request your records below.

Access your health records through MyChart

The MyChart patient portal is the simplest and most convenient way to access your medical information. Whether you’re at home, at work or on the go, you can get test results, prescription details, appointment information, after-visit instructions summary notes and more.

Help manage a family member's health

If you have an active role in managing someone else’s health care, whether it’s your child (under age 13), parent or other adult, MyChart proxy access gives you access to the health information and tools you need to help take care of those who depend on you.

Other ways to request your medical records

The method for obtaining records depends on the location where care was delivered and who received care (yourself or another patient, such as a minor). Please follow the detailed instructions below.

Hospital, clinic and urgent care

There are two ways you can request copies of your MultiCare hospital, clinic or urgent care medical records — digitally or by mail. Washington state law allows up to 15 business days to respond to a medical records request; in most cases your request will be processed within five business days.

For additional help, contact MultiCare Health Information Management by phone at 855-673-2673, Monday through Friday from 8am – 4:30pm. (Note: The department is closed on all major holidays.) When needed, you may also stop by one of our medical records locations in person.

You may only request copies of your own medical record through the patient portal. If you are requesting records on behalf of another patient, including your minor children, please follow directions outlined in option 2 below.

You may also send a signed and dated letter specifying what records are to be released and to whom.

After completing and signing the form, please return the form by email (scanned in), fax or mail. The fax is monitored Monday through Friday from 8am – 5pm.

Behavioral health (outpatient) and substance abuse

There are two ways you can request copies of your MultiCare outpatient behavioral health and/or substance abuse medical records — digitally or by mail. Washington state law allows up to 15 business days to respond to a medical records request.

For additional help, contact MultiCare Health Information Management by phone at 855-673-2673 on Monday through Friday from 8am – 4:30pm (Note: The department is closed on all major holidays).

Medical imaging

Medical records for medical imaging exams — CT, MRI, and X-ray — are processed through MultiCare’s central medical imaging records department, open seven days a week from 7am – 11pm. There is no cost for requesting or receiving copies of your medical imaging records.

Surgery center

Medical records are processed through Atlas Healthcare Partners, the managing partner for MultiCare Surgery Center locations in Auburn, Puyallup and Yakima.
