Whether you are recovering from an illness or injury, managing a serious health condition or are making end-of-life choices, our home health and hospice services can help. Working closely with your doctors and the rest of your care team, our specially trained health professionals provide compassionate care no matter where you are: at home, an adult family home, a retirement home or an assisted living facility (hospice services are also available to you if you are in the hospital).
- Disease management education
- Home-based rehabilitation
- Home safety evaluations
- Foley catheter changes
- Ostomy care
- Post-operative care, including knee and hip replacements
- Telemonitoring for patients to monitor their vitals
- Wound care
- Intermittent bathing assistance
- Balance retraining and fall prevention
- Assistance with personal care, such as bathing and grooming
- Bereavement support for loved ones
- Comfort therapies including massage, reiki, music and aromatherapy
- Emotional and spiritual support
- Medical equipment, supplies and medications, as needed for comfort
- Pain and symptom management
- Physical, occupational and speech therapy
- Volunteers who can provide respite to caregivers or provide additional comfort and companionship to patients
MultiCare Home Health and MultiCare Hospice do not provide round-the-clock care, but can offer resources to help you access that care if needed.
