TACOMA, Wash. – Innovaccer Inc., a leading health care technology company, has announced a collaboration with MultiCare Connected Care (MCC), one of the leading clinically integrated care networks in the state of Washington, to improve the health of populations in the Pacific Northwest. This collaboration will focus on implementing a care management framework for MCC’s Medicare Shared Savings Program (MSSP) Track 1+ patient population. MultiCare Health System is a minority investor in Physicians of Southwest Washington (PSW).

MCC and PSW have partnered to improve health care delivery and management and lead the way to healthier futures. With Innovaccer’s Data Activation Platform, MCC plans to enhance its existing operational capabilities and capacities for population health. MCC will be deploying the Data Activation Platform to unify the medical records of more than 37,000 Medicare patients, gathered from multiple data sources and will utilize them to manage beneficial health initiatives ensure the wellbeing of the population.

In addition, Innovaccer’s population health analytics tool, InGraph, will enable MultiCare Connected Care to monitor utilization across the network and reduce costs for its employed and independent providers. MCC will also be implementing InCare, Innovaccer’s artificial intelligence-assisted care management, to ensure real-time care coordination among clinicians to equip them with efficient automated workflows.
