Prepare for your initial MultiCare Behavioral Health assessment and cut down on wait time by completing your intake packet before you arrive.

Please print out the complete packet, fill out all of the pages no more than 5-days prior to the assessment, sign and date where appropriate.

If the assessment is scheduled in the office:

• Please bring these with you to your assessment along with your identification and insurance information.

If the assessment is scheduled as a virtual appointment:
