Is Advanced Primary Care right for me?

If you have two or more health issues (chronic conditions) that you manage daily, call us and we’ll talk through your options and insurance plan eligibility.

How do I become a patient or sign up a family member?

Call us at 509-530-4260 and we’ll discuss your health needs, check your insurance eligibility and make a plan to transition care from your current primary care provider.

Who is on my care team?

Your care team will provide customized support, connections to additional care and appointment scheduling. They’ll also make sure you understand your treatment plan and help you manage and pay for medications. Your care team includes:

  • Physicians and advanced practice providers
  • Nurse care managers
  • Registered dietitian
  • Social worker
  • Behavioral health specialist
  • Medical assistants
  • Front desk staff

How will this team work with my specialists?

When specialty care is needed, your care team will help set up appointments, follow up with specialists and incorporate results into your care plan. This ensures there are no gaps in communication or treatment.

If you already see specialists, such as a heart doctor or ENT, we’ll make sure we’re connected to them.

How do you coordinate my medications and costs?

Your care team will work with MultiCare Specialty Pharmacy to help optimize your prescriptions, give tips on how to best manage refills and ensure you’re taking medications correctly. If you have difficulty paying for medications, they can help you find financial assistance and other programs.

What’s the difference between this service and traditional primary care?

MultiCare Advanced Primary Care is a specialized clinic for those living with multiple chronic health conditions, such as diabetes, high blood pressure or heart disease. These can take a lot of time and energy to manage.
