Patient Transfer Center

The MultiCare Mission Coordination Transfer Center is the system-wide point of contact for all adult patient transfers and direct admissions for all MHS hospitals that serves as a hub to improve patient safety, patient and provider satisfaction, provide an overall improved experience for providers and referral facilities, create standard processes that increase throughput and decrease the time and effort required to mobilize patients into the MultiCare Health System to receive definitive care.

Patient Transfer Center Process

  1. Transferring/referring providers or facilities call 855-MHS-3030 (855-647-3030)
    • This number is manned 24/7 – 365 by dedicated and experienced Intake RNs.
  2. The Intake RNs will discuss the patient’s needs with the sending provider/facility to determine the service lines, level of care, and the most appropriate MHS facility where that patient can receive definitive care.
    • Referring facility will push all images to MHS as appropriate.
    • Referring facility will fax all treatment records for current admission to: 253-792-6764
  3. After the initial referral call, the Intake RN will target the best and most appropriate facility based on geography, medical and physical capability, and capacity.
  4. The intake RN will contact the appropriate consulting and/or admitting provider(s) and facilitate a conference call with the referring provider and consulting/admitting MHS provider on a recorded line.
  5. After medical approval is obtained via the conference call or other avenue, the Intake RN will notify the referring facility of the patient acceptance and the MHS facility that accepted the patient.

The MHS Mission Coordination Transfer Center is a System-wide patient transfer center that coordinates all Adult Patient Transfers into MHS, between MHS facilities, and Adult Direct Admissions to a MHS facility.

We do not support transfers for the following:

  • Pediatrics
  • NICU
  • Behavioral health
  • Trauma patients less than 15 years of age
  • STEMI – (Not a standard function of Transfer Center, but our team will assist with facilitating contact with the closest MHS facility with a Cath Lab when the referring location’s standard pathway is unavailable)
