Take control of your health with MultiCare MyChart Care Companion, an interactive tool designed to keep you connected and on track with your care plan.

Your care team will be with you every step of the way by tracking your progress and answering your questions. Log in to MyChart to view tasks, complete check-ins and manage reminders associated with your care journey.

Frequently asked questions

Yes, you’ll need a MyChart account to participate in a care journey via MyChart Care Companion. You can create an account in several ways:

  • Through clinic staff, who can sign you up directly while you’re at the front desk or in the exam room
  • With a MyChart activation code, found on your After Visit Summary, a billing statement or via text message after you come in for a visit
  • Via self-sign-up online by matching your information against what’s on file in your medical record or with third-party identity verification

Once you have an account, you may access it by downloading the app to your mobile device from the Apple App Store, Google Play or your computer.

Your provider will enroll you in Care Companion if you meet the criteria for the program.

There are several ways to access care journey tasks in MyChart:

  • You may receive a notification or email when a task is due. Select it to be directed to the To Do section in MyChart.
  • Select View tasks in the reminder section of your MyChart dashboard.
  • From your MyChart dashboard screen, select Menu, then To Do.

Yes, you can share your care journey with a family member or friend by granting them proxy access.

If your procedure is rescheduled, the care plan tasks will be updated automatically when your provider enters the new procedure date in your health record. If your procedure is canceled, you’ll be removed from the care journey and no longer receive notifications. If your condition changes, the care plan will be updated or removed based on your provider’s recommendations. And if the care journey is reapplied, you will start from the beginning of the plan.

If at any point you wish to discontinue Care Companion, you can withdraw from a care journey by using one of these methods:

  • Message or call your provider and request that they remove you from the program
  • After the procedure date, once all tasks are overdue, the care journey will automatically discontinue and be removed from your health record
