Washington Labor & Industry Instructions
This page includes care instructions for employees of businesses covered by L&I. Unlike other types of insurance, L&I only cover injuries if they happen at a definite time and place at work. Also, claims for occupational diseases are accepted only if your work and medical history shows you have an illness or infection that was directly caused by the work you do, and not by something else.
Multicare Occupational Medicine will be contacting you to set up an appointment. If you do not hear from us within 24-48 hours, please contact us at 253-459-6541 to set up an appointment at your convenience.
- Be sure that your employer is aware of the injury and that any paperwork the employer needs is completed.
- Keep the paperwork provided to you today. The forms have your assigned L&I claim number, which you will need.
- Review the Worker’s Compensation Benefits pamphlet. It outlines the process and provides important information about your care.
- Washington State L&I will provide you with additional information regarding your claim when/if your claim is allowed.
- If additional medical treatment is needed be sure to tell your healthcare providers that a work injury claim has been filed and give them your assigned claim number.
- Go to the Washington State Department of Labor and Industries website to get online access to the claim information and other important information about your care.
- COHE (Center of Occupational Medicine Education) is a program through Washington State L&I that helps coordinate care and support recovery for injured workers. You may ask you provider for your COHE representative contact information or visit Centers of Occupational Health & Education (COHE)
- Should your Washington State L&I worker’s compensation claim be denied or rejected you will be financially responsible for any future services which are not covered/excluded by any private insurance in which you are enrolled, including any copay or cost shares. If you do not have any private insurance, you will be responsible for the billed amount.
