Meet the Advanced Primary Care Team

With a supportive team by your side, you can expect one-on-one, coordinated care to help you manage your complex health needs.

  • Physicians and advanced practice providers — Our physicians and advanced practice providers will go over your health goals, reconcile your medical history and review your medications during your first appointment. Together you’ll develop a personalized treatment plan with a focus on your overall health and well-being.
  • RN care managers — Care managers will ensure your care is connected every step of the way. Expect to connect with them to coordinate labs, imaging and specialist care.
  • Registered dietitian — A healthy diet is key to maintaining your well-being. Our dietitian will help you make a plan that works best for you and your goals.
  • Behavioral health specialist — Life can be overwhelming and difficult to navigate when you’re going through a tough time. Our behavioral health specialist can help walk you through that difficult time and bring comfort with talk therapy.
  • Social worker — Similar to our behavioral health specialist, our social worker can provide resources and assistance within the community that patients don’t always know is available. Social workers also assist with scheduling appointments and transportation.
  • Medical assistants — Prior to seeing your physician or advanced practice provider, our medical assistants will get you settled into a room, retrieve your vitals and talk with you about why you are here today. If the physician asks for labs, the medical assistant will draw blood and package it up to be picked up by our courier and sent to the lab to be tested.
  • Front desk staff — Your first point of contact when you walk into a clinic should be enlightening and comfortable. Our patient experience representatives will make sure you get into the clinic safely, offer you coffee or tea, and answer any questions you may have prior to seeing your physician.
  • Educators — At Advanced Primary Care, it’s important that we educate our patients as much as possible about chronic complex illnesses. We have nurse educators, diabetic educators, exercise class instructors and other health resources available to established patients.
