Program: Pathways to Healthy Living
The grant will allow Catholic Charities Housing Services (CCHS) to coordinate with community partners and their Resident Services Program to provide information and referral services through one-on-one high-level engagement with residents at 11 multi-family and senior housing sites. Activities for this grant include coordinating on-site health and resource fairs and health screenings, as well as nutrition classes, illness prevention classes and presentations, referrals to mental health services, and connection to social supports for residents and invited guests living at our housing sites.

People For People operates the Yakima County Meals On Wheels program to provide vulnerable seniors with healthy meals, friendly visits and safety checks. Funds will deliver nutritious meals to vulnerable seniors that need home delivered meals.

Rod’s House
Program: Case Management and Advocacy for Young People Experiencing Homelessness
Rod’s House works to break the cycle of homelessness by ensuring young people’s basic human needs are met and that they have a safe, stable home; permanent, positive connections; meaningful education and employment opportunities; behavioral and physical health care; and supports that reinforce their individual abilities. Funds would be used to support case management efforts as they guide youth and young adults to improve their educational and/or economical potential, and secure counseling and behavioral health support. Funds will also provide hot meals, showers and access to temporary housing within two shelters.

Program: Expanding Outpatient Mental Health Services for All Ages in Yakima County
Triumph Treatment Services provides a multifaceted, integrated continuum of care with a mission to serve the underserved in Yakima County and the Central Washington region. The agency provides resources and services for individuals and families who experience substance use disorder, mental health issues, homelessness and poverty. Funds will be used to provide an additional licensed mental health counselor.

The Wellness House project addresses the community’s need for comprehensive support services for patients and their families facing life-challenging illnesses. Funds will be used for support services and program operations, and will provide resources to assist in the well-being of their patients, marketing and outreach efforts, and evaluation and monitoring to measure the impact and effectiveness of the program.
