Medical Records for Rockwood Clinic Patients

There are three ways you can request copies of your MultiCare Rockwood Clinic medical record — fax, mail or in person. For additional help, contact the Release of Information Department by phone at 509-838-2531. For inquiries about records for medical imaging exams, please call 509-342-3581.

To request your records, please print and complete a medical records release form. We have included instructions on how to complete the release form in the downloadable file.

After completing and signing the form, please return the form by fax or in person.

Processing time and fees

MultiCare Rockwood Clinic has partnered with Ciox Health, the nation’s largest provider of medical record release of information services, to process and fulfill your request for a copy of your medical record. While we strive to make medical records copies available within three business days, please allow up to 15 business days for your request to be processed.

A reasonable fee charged is charged by Ciox Health for this service, and pre-payment is required. Due to the strict procedural and highly regulated steps required (known as the release of information process), a fee is charged for this service as outlined below:

  • Electronic records delivered in electronic format: $6.50
  • Electronic medical record with paper records, with paper records delivered in electronic format: $6.50 + $0.07 per page, labor cost to create and deliver the medical record
  • Electronic records delivered in paper: $0.90 per page labor cost plus $0.05 per page for paper and toner
  • Paper records delivered in electronic format: $0.07 per page labor fee
  • Paper records delivered in paper format: $0.12 per page
  • Medical imaging records put onto a CD: Per patient request, an additional $6.50 will be charged for images that are put onto a CD

The charge for copying medical records helps cover cost of labor and supplies, and is mandated by the Washington State Legislature in RCW 70.02.
