Medical Records for Valley Hospital Patients
There are three ways you can request copies of your MultiCare Valley Hospital medical record — fax, mail or in person. For additional help, contact the Health Information Management Department by phone at 509-603-5431.
To request your records, please print and complete a medical records release form. We have included instructions on how to complete the release form in the downloadable file.

The Health Information Management department is open Monday through Friday from 8am – 4pm and is located on the first floor of Valley Hospital near the Medical Imaging department.
Processing time and fees
MultiCare Valley Hospital contracts Ciox Health, a professional medical record copying service, to ensure that copies of your medical record are available as quickly as possible. While we strive to make medical records copies available within three business days, please allow up to 15 business days for your request to be processed.
A reasonable fee charged is charged by Ciox Health for this service, and pre-payment may be required. The charge for copying medical records helps cover cost of labor and supplies, and is mandated by the Washington State Legislature in RCW 70.02. Ciox Health will contact you directly with a pre-payment amount. Medical records are not released until pre-payment is received.
We often recommend that you only request copies of the pertinent parts of your medical record in order to limit the number of pages copied. This typically includes:
- All medical reports documented by your provider (examples include discharge summary, operative reports, consultation reports, etc.)
- Relevant progress notes from your visit and diagnostic tests (examples include labs, medical imaging, EKG, etc.)
